A Commitment to Caring

Even though your new puppy is active and appears healthy, it is important to establish a relationship with a veterinarian as soon as possible. In addition to improving your new puppy's socialization skills, this also allows your veterinarian to become a trusted partner in your puppy's upbringing.

Timely health care is one of the most loving and responsible things you can do for your new pet. Newborn puppies will receive antibodies to fight disease from their mother's milk. After weaning, this immunity begins to decrease. After this time, vaccinations (shots) will help to protect your puppy by introducing mild doses of disease-causing agents into his body to stimulate him to produce his own antibodies.

A vaccination schedule through your puppy's first six months is necessary to protect him through the early, most susceptible time of his life. These first months are critical to the health and well-being of your new pet. Let our comprehensive Puppy Plan alleviate the worry by providing everything necessary for your newest family member.

Below is a typical schedule for routine Puppy Plan visits for the included elective services.

Visit #1 ( 8 weeks of age)

  • Complete Physical Examination
  • Fecal Examination
  • DHLP-P & Corona Vaccination (first puppy shots)
  • Deworming (if necessary)

Visit #2 ( 10 weeks of age)

  • DHLP-P & Corona Vaccination
  • Deworming (if necessary)
  • 3 month supply of heartworm preventative

Visit #3 ( 12 weeks of age)

  • DHLP-P Vaccination
  • Rabies Vaccination
  • Fecal Examination
  • Deworming (if necessary)

Visit #4 ( 14 weeks of age)

  • DHLP-P Vaccination
  • Deworming (if necessary)

Visit #5 ( 16 weeks of age)

  • DHLP-P Vaccination
  • Fecal Examination
  • Deworming (if necessary)

Visit #6 ( 6 months of age)

  • Parvo Virus Vaccination
  • Fecal Examination
  • Deworming (if necessary)
  • Microchip (Home Again) Implant *
  • 10% Discount on Spay / Neuter Cost


The fee for the Puppy Plan is $ 300.00. A minimum payment of $ 100.00 is required on the first, second and third visits. In addition to the services outlined, any additional examinations, fecals or dewormings, are included as no charge items under the plan until your puppy is six months old.

The plan does not include medication, hospitalization, surgery, laboratory other than specified, and x-rays. It is the owner's responsibilty to inform the staff of Polk County Animal Hospital if any services covered under the Puppy Plan are not wanted. In the event a plan is cancelled by the owner prior to completion, the hospital reserves the right to adjust the owed balance according to the individual services rendered up to the point of termination.

The fee for the sterilization surgery is NOT included in the cost of the plan. A 10% discount off the cost of the surgery will be issued if scheduled before the pet is 6 months of age. Failure to schedule this surgery prior to 6 months of age will result in the forfeiture of the sterilization discount. Services and products are not transferable.
